The application shall be on a form prescribed by the secretary of state and shall include: The act to be referred; The designation of a committee of three (3) applicants who shall represent all sponsors and subscribers in matters relating to the referendum; The signatures and addresses of not less than one hundred (100) sponsors who shall be qualified registered voters; and A statement that the committee of applicants are qualified registered voters who signed the application with the proposed act attached.
